AAHKS Member Tool: Free Edition

TJA Facility & Compensation Value Calculator FREE

National-average estimates of what your total knee and hip replacement cases are worth to a facility, how CMS's CY2027 proposal changes your wRVU compensation, and how employed vs. collections-based pay compares. Free tells you whether to negotiate. The paid edition uses your own hospital's wage index, IME, DSH, and payer mix instead of national averages.

§1

Facility Value Per Case

National-average Medicare payment by site of service, CY2026 vs. CY2027 proposed. Shown as a range reflecting typical hospital-to-hospital variation nationally (roughly the middle 50% of U.S. hospitals by wage index). This is just an estimate. The paid version has more exact numbers to use for negotiation, built from your own hospital's wage index instead of a national range.

Estimate, not an offer. National averages only, not specific to any hospital. DRG 469/470 figures exclude IME, DSH, uncompensated care, and outlier payments, which are hospital-specific and only available in the paid edition.
§2

wRVU Per Case, CY2026 vs. CY2027

CMS's proposed CY2027 rule cuts work RVUs for TKA and THA. Enter your own $/wRVU rate to see the dollar impact per case.

CY2026 work RVU is a single blended figure in this model (not yet split by procedure); CY2027 is CMS's proposed procedure-specific value. Estimate, not an offer.
§3

Surgeon Share of Total Episode Payment

Matches AAHKS's published methodology: Surgeon Share = Surgeon Professional Fee ÷ (Surgeon Professional Fee + Facility Fee), both at national-average Medicare rates. Percentages only. See the disclaimer above for why dollar figures aren't the point of this argument.

Employed vs. Collections-Based Compensation

Two common physician compensation models, compared at your own case volume and assumptions.

ModelComponentAnnual Comp, CY2026Annual Comp, CY2027Δ
Each TJA case assumes 2 post-op office visits, which are bundled into the surgical global period (not separately billable) but still occupy office schedule slots. Those slots are backed out of your weekly office volume before counting billable E/M encounters. Collections-based model = national-average Medicare RVU × conversion factor × your payer mix multiplier (1.3× approximates a typical commercial/Medicare blend; adjust to your own practice) × your chosen % share. Ancillary income (ASC ownership, imaging, DME, PT, etc.) isn't calculated. It's practice-specific and depends on ownership/buy-in status, not CMS payment data, and doesn't apply to the Employed model since ancillary ownership distributions are a private-practice/partnership structure, not something a W-2 employed physician typically holds. Enter your own figure if applicable. Estimate, not an offer.
